The Healthy Lifestyles Health Assessment

 UMS is offering a financial incentive to motivate you to learn what you can do to improve your health.  Anthem’s Healthy Lifestyles Health Assessment is an online questionnaire that will:

  • help you to understand your overall health status,
  • provide you with a personalized report to identify your risks for developing many preventable conditions,
  • suggest action steps using an interactive dashboard that literally shows you the effects of changing your lifestyle,
  • link you to health coaches, Anthem programs and resources that can help reduce your health risks and help you keep your commitment to better health.

Whether you think you’re perfectly healthy or know you have a few areas you can work on, taking the Healthy Lifestyles Health Assessment may be the smartest thing you can do for your health.  Our goal is to enable employees to improve their health and the health of their families that could lead to a decrease in the escalating cost of our medical plan. 

 Your personal data and results are completely confidential and can never be accessed by UMS.

Complete the Healthy Lifestyles Health Assessment between February 14 and April 30, 2010 (June 4th for PATFA) to receive a one-time $100 (less applicable payroll taxes) incentive credit towards your share of your UMS health insurance premium and an additional $100 (less applicable payroll taxes) credit for your spouse/domestic partner who is covered by the University health insurance plan and who also does so voluntarily.   To receive the financial incentive*, you and/or your spouse/domestic partner must complete the entire Health Assessment and must indicate a willingness to accept health coaching by telephone for health risks identified by the Assessment. The incentive will appear in your paycheck.

*This incentive program is available to all represented and non-represented employees and their spouses or domestic partners who are enrolled in a UMS health plan.   

If the employee and/or their covered spouse/domestic partner completes 1 and 2 listed below, the employee will receive a one-time one hundred dollar ($100) (less applicable payroll taxes) incentive for themselves plus an additional one-time one hundred dollar ($100) (less applicable payroll taxes) incentive for their spouse/domestic partner (if applicable) towards employee health premium contributions for calendar year 2010.

It's Easy To Get Started:

1. Employees and/or their spouse/domestic partner will need to obtain key bio-metric data (e.g. BP, BMI, lipids); click here for a handy Checklist 

  • UMS will run on-campus "know your numbers" screenings for bio-metric data (completed), OR
  • Employees and/or spouse/domestic partner can have tests done by their own doctor;

2. Employees and/or their spouse/domestic partner voluntarily complete the Anthem's Healthy Lifestyles Health Assessment 

  • Assessment must be entirely completed between February 14, 2010 and April 30, 2010 (June 4th for PATFA)
  • Employee and/or spouse/domestic partner must indicate a willingness to accept telephonic coaching (MUST answer "Yes" to final question of the Assessment)
